Transport Drivers Must Have Case Number
Before dispatching your driver, make sure they have at minimum, the decedent's name AND case number.
MAIL CHECKS ONLY, DO NOT FAX COPIES OF CHECKS. THE DEATH CERTIFICATE WILL BE TRANSFERRED TO THE MORTUARY LISTED ON THE RELEASE AUTHORIZATION ONCE REMOVAL IS COMPLETE.
